What to bring to first Puppy/Kitten Visit:

Any records you may have

Recent stool sample (ideally within the last 24 hours)

Your new puppy/kitten :-)

What you will go home with:

A Portage Animal Hospital Puppy Pack which includes:

Informational brochures

A sample of Iams food

A toothbrush and a sample of toothpaste

A shampoo sample

Free first dose of monthly heartworm/intestinal parasite preventive.

Welcome New Clients!!

First visit check list:

1. New Client Form please come in a few minutes early to complete a new client form. 

2. Records if your pet has any vaccination records or records of previous treatment please bring those with you. If you would like, we can contact your previous vet and have records faxed to us @ 269.329.7016

3. Fecal sample please put a stool sample in a plastic bag and bring it with you so that we can check for intestinal parasites.
