First visit check list:

1. New Client Form please come in a few minutes early to complete a new client form. 

2. Records if your pet has any vaccination records or records of previous treatment please bring those with you. If you would like, we can contact your previous vet and have records faxed to us @ 269.329.7016

3. Fecal sample if your pets cooperates please put a stool sample in a plastic bag and bring it with you so that we can check for intestinal parasites.

Portage Animal Hospital happily welcomes new clients and pets.  We serve clients in Portage and surrounding cities such as Kalamazoo, Schoolcraft, Scotts, Vicksburg, Three Rivers, and Galesburg. We see dogs, cats, rabbits, guinea pigs, rats, mice, and chinchilla's etc.  If you have any questions about our practice, please call our office for personal attention.

Payment is due at the time of service.  We accept cash, check, and all major credit cards.
